BACKGROUND Dysfunction of cerebral autoregulation might contribute to neurological morbidity after cardiac surgery. In this study, our aim was to assess the preservation of cerebral autoregulation after cardiac surgery involving cardiopulmonary bypass (CPB). METHODS Dynamic and static components of cerebral autoregulation were evaluated in 12 patients undergoing coronary artery bypass graft s...
BACKGROUND The authors tested the hypothesis that during laparoscopic surgery, Trendelenburg position and pneumoperitoneum may worsen chest wall elastance, concomitantly decreasing transpulmonary pressure, and that a protective ventilator strategy applied after pneumoperitoneum induction, by increasing transpulmonary pressure, would result in alveolar recruitment and improvement in respiratory ...
In 1895 Trendelenburg described his sign to determine the integrity of hip function. We found the sign to be positive in a patient whose hip was clinically and radiologically normal, and therefore investigated this in other patients. We confirmed that a medial shift of the mechanical axis of the leg below the hip may cause a positive Trendelenburg sign. This has not been previously described.
Patients with Legg-Calvé-Perthes disease (LCP) often exhibit relative overgrowth of the greater trochanter and shortening of the femoral neck. Biomechanically, this corresponds to a shorter lever arm and a decreased muscle tension which may result in a Trendelenburg gait and pelvic instability. This is a retrospective study of 31 patients (32 hips) with LCP disease and relative overgrowth of th...
OBJECTIVE To determine whether the 15-degree reverse Trendelenburg position (RTP) during functional endoscopic sinus surgery improves endoscopic field of view and reduces intraoperative blood loss when compared with the horizontal position (HP). STUDY DESIGN A prospective, randomized controlled trial. SETTING St Paul's Sinus Centre, Vancouver, Canada. SUBJECTS Patients with chronic rhinos...
The study purpose was to evaluate preventive use of dorzolamide-timolol ophthalmic solution (Cosopt) during laparoscopic surgery with the patient in steep Trendelenburg (ST) position. Periorbital swelling, venous congestion, and elevated intraocular pressure (IOP) may produce low ocular perfusion. Prompt IOP reduction is important because 30- to 40-minute episodes of acute IOP elevations can re...
Postoperative alopecia is an uncommon complication and its outcome is aesthetically drastic. Although its mechanism has not been clearly elucidated, a proposed risk factor is steep Trendelenburg positioning (30-40 degrees) that is frequently used during robotic gynecologic surgeries. We report a case of postoperative alopecia in 53-year-old female patient who had undergone robotic-assisted lapa...
OBJECTIVE To investigate which of two commonly used treatment protocols for exercise-associated postural hypotension (EAPH) resulted in earlier discharge from the medical facility. METHODS This randomised clinical field trial was undertaken at two Ironman Triathlon competitions and one ultra-distance footrace. All collapsed athletes admitted to the medical facilities were considered for the t...
