The goal of this study was to establish a reliable method to evaluate systemic bioavailability and to determine equisystemic effects (microgram dose producing equal systemic cortisol suppression) of inhaled corticosteroids (ICS). Steroid naive asthma subjects (n = 156) were enrolled at six centers. A 1-week doubling dose design was used for each of six ICS and matched placebos for a total of fo...

The use of corticosteroids for the treatment of equine joint disease, as well as the controversy surrounding the risk–benefit ratio of their use have been reported for almost five decades. Few, if any, of the early clinical reports suggesting deleterious effects associated with intra-articular (IA) corticosteroid administration have withstood close modern-day scientific scrutiny. Specifically, ...

Most patients with Reiter's syndrome (RS) or B27associated conditions respond well to non-steroidal anti-inflammatory agents. Hence there is no need for corticosteroid therapy or immune modulation in mild cases. Aggressive treatment should be considered only if a patient develops severe systemic manifestations such as vasculitis or iridocyclitis or the disease has a prolonged, crippling course ...

Introduction Both Sweet’s syndrome and pyoderma gangraenosum are neutrophilic dermatoses with abrupt onset of painful skin lesions. In Sweet’s syndrome systemic symptoms such as fever, headache and leucocytosis together with painful skin papules are often seen. Corticosteroid therapy is often sufficient treatment. Pyoderma gangraenosum is characterized by the development of painful ulcerative s...

KEY CLINICAL MESSAGE Monocytosis is often profoundly implicated in chronic myelomonocytic leukemia (CMML). We diagnosed a 63-year-old woman with CMML involving heterochronological systemic polyserositis with infiltrating benign monocytes. The patient's pleural effusion was eliminated completely with corticosteroid therapy. We speculate that our case represents an immunological reaction with act...

In this series a very large therapeutic dose of corticosteroid was administered by retention enemata. For the first week the author gave the equivalent of between 300 and 750 mg. of cortisone daily and for the next three weeks 150 to 375 mg. daily. The results were excellent but systemic effects were noted in all who received more than the smallest dose used.

Management of corticosteroid (CS) hypersensitivity is not easy, and the choice of an alternative CS is difficult, owing to the high frequency of cross-reactions. Deflazacort has been reported to be a safe alternative in subjects with CS hypersensitivity (1), and reports of deflazacort hypersensitivity are rare (2–7). Here, we report a difficult-to-manage case of systemic allergic dermatitis cau...

We report a patient who presented with an association between propylthiouracil treatment and recurrent episodes of an adult respiratory distress-like syndrome (ARDS). The bouts of ARDS were associated with a generalized reaction suggestive of systemic lupus erythematosus (SLE), responsive to corticosteroid therapy.

We report a case of Riedel's thyroiditis presenting with a systemic illness, life-threatening stridor and a stony hard goitre. Diagnosis was confirmed by open thyroid biopsy. Treatment with corticosteroid resulted in a dramatic improvement. A possible autoimmune mechanism in the pathogenesis of Riedel's thyroiditis is discussed.
