We introduce a 28-year-old woman with Thalassemia major whose clinical assessment, including two-dimensional Doppler echocardiography demonstrated severe left ventricular hypertrophy with severe biventricular enlargement and systolic dysfunction as well as severe diastolic dysfunction. We hereby address these issues from an echocardiographic point of view.

A 37-year-old woman developed Wernicke encephalopathy after prolonged psychogenic food refusal. MR revealed characteristic signal abnormalities in the midbrain and dorsal thalamus. Follow-up scans showed atrophy and third ventricular enlargement. Wernicke encephalopathy can occur in nonalcoholics, and MR imaging is useful in both the diagnosis and follow-up.

The echocardiographic features of a patient with severe aortic regurgitation due to a "floppy" aortic valve are presented. The salient abnormality observed was marked fluttering of the aortic valve cusps in diastole. Fluttering of the anterior leaflet of the mitral valve and left ventricular enlargement were noted. The diagnosis of a "floppy" aortic valve was substantiated at surgery.
