中咽頭は小唾液腺癌の発生部位のひとつであるが，その診断には難渋することがある。Polymorphous adenocarcinoma（以下，PAC）は小唾液腺に好発する唾液腺癌である。今回，口蓋扁桃腫瘍を呈したPACを経験した。症例は57歳女性。咽頭違和感を主訴に近医を受診した際に左口蓋扁桃腫瘍を指摘され，当科を紹介受診した。CTで左口蓋扁桃に潰瘍を伴う腫瘍を認めた。全身麻酔下の生検でPACと診断され，再建手術が施行された。術後病理で正常な口蓋扁桃組織は残存しており，扁桃窩の小唾液腺由来の腫瘍が口蓋扁桃腫瘍を呈したと考えられた。口蓋扁桃腫瘍の鑑別として唾液腺癌も考慮する必要がある。
