This case report presents an uncommon case of maxillary molar with two palatal root canals and an enamel pearl in the furcation area. The article discusses root canal complexities of maxillary second molars as well as possibility of coexisting anomalies in the region that makes radiographic interpretation difficult and compromises the success of endodontic treatment.

The most difficult maxillary teeth for endodontic treatment are the maxillary first molars (MFM) due to their complex root canal anatomy. The presence of two roots and C-shaped canals in MFMs has been reported in rare cases. A patient with multiple anomalies of the maxillary teeth, including shovel-shaped incisors, talon cusp, bilateral dens invaginatus and bilateral peg-shaped supernumerary incisors is reported. The patient also exhibited Carabelli's cusp on both maxillary first molars. No developmental syndrome was identified. This very unusual combination of anomalies has not been reported previously.
