Dengue virus infection affects the heart structurally and functionally. Clinical manifestations of cardiac complications secondary to dengue virus infection vary from self-limiting arrhythmias to severe myocardial infarction, leading to hypotension, pulmonary edema, and cardiogenic shock. However, we report a case of dengue hemorrhagic fever (DHF) complicated by a complete heart block. A female...
Reperfusion therapy with tissue plasminogen activator (tPA) is a rational therapy for acute ischemic stroke. Properly titrated use of tPA improves clinical outcomes. However, there is also an associated risk of hemorrhagic transformation after tPA therapy. Emerging data now suggest that some of these potentially neurotoxic side effects of tPA may be due to its signaling actions in the neurovasc...
Volume 91, Number 2 www.aafp.org/afp American Family Physician 123 An 84-year-old woman presented with a one-week history of fever, blistering lips, and progressive skin sloughing. She had diffuse muscle aches and increased facial pigmentation and edema without recent sun exposure. Her history was significant for removal of a toenail by a podiatrist three weeks earlier. The podiatrist prescribe...
OBJECTIVE Analyze the cases of posterior reversible encephalopathy syndrome (PRES) admitted in a Neurology Department during an 8-year period. METHOD Retrospective observational study in a central hospital in the north of Portugal. RESULTS 14 patients were identified, mean age 52.3 years. Precipitating factors included: eclampsia, isolated arterial hypertension, spinal trauma and autonomic ...
Reperfusion with recombinant tissue plasminogen activator (tPA) sometimes causes catastrophic hemorrhagic transformation (HT) in the ischemic brain. Consequently, the application of tPA has been strictly limited. Recent studies have indicated that matrix metalloproteinases (MMPs), especially MMP-9, play a critical role in blood brain barrier (BBB) disruption in the ischemic brain, leading to br...

A ngiogenesis, the regrowth of new blood vessels, occurs after stroke 1 and thus raises hopes that this regeneration is beneficial for patients with stroke. There is no doubt, that the restoration of blood supply, providing oxygen and nutrients to the ischemic brain tissue, is beneficial for stroke recovery. However, it is debated, whether angiogenesis truly contributes to improved recovery. Th...
An 8-year-old boy presented at UCLA Hospital with a one month history of hypertension prior to suffering a sudden onset of acute abdominal pain, rectal bleeding, peritonitis, and shock. Sigmoidoscopy showed diffuse mucosal friability. At laparotomy, inflammation and edema of the entire colon and terminal ileum were detected with two necrotic areas on the cecum. A 5 cm right adrenal pheochromocy...
We report the occurrence of acute pulmonary edema in a patient receiving therapy with nalbuphine, a synthetic narcotic analgesic. Other potential causes of acute pulmonary edema were excluded. It is likely that all opiate drugs share the propensity to produce acute pulmonary edema, and nalbuphine resembles other opiates in this regard.
