症例は64歳，男性．S状結腸癌（S，cT3N0M0，cStage IIa）の診断で腹腔鏡下S状結腸切除術（DST吻合）を施行した．特に合併症なく第7病日に退院したが，第24日病日に腹痛を主訴に当院を受診した．来院時に施行した腹部単純X線写真では下行結腸を起点に著明な結腸の拡張を認めた．腹部造影CTでは吻合部口側の結腸が吻合部に嵌頓していたため，DST吻合部の腸重積と診断した．内視鏡的整復も困難であったため，同日腹腔鏡下で緊急手術を行った．術中所見では口側結腸が吻合部に強固に嵌頓していたため，吻合部の切除・再吻合（DST吻合）を行った．再手術後は合併症なく第31病日に退院となった．術後病理検査では粘膜面の浮腫や線維化を認めたものの，腸重積の先進部となる器質的病変はなかった．腹腔鏡下S状結腸切除後のDST吻合部腸重積の症例は稀な病態であると考えられ，若干の文献的考察を加えて報告する．
