症例は71歳男性，咳嗽を主訴に近医を受診．胸部X線写真上，右上肺野に巨大な腫瘤を認め当院へ紹介受診となった．他院で9年前に右肋骨腫瘍を指摘されていたが，2年で経過観察から外れていた．胸部X線写真上，9年前の腫瘍は8.4×6.5 cmであったが，受診時は16.4×13.4 cmであった．後側方切開で手術を施行．第2肋骨全摘および第3肋骨の部分切除を行い，腫瘍を摘出した．術後合併症は認めなかった．術後病理検査でも骨線維性異形成で，悪性転化は認めなかった．骨線維性異形成は一般的な良性腫瘍で，無症状なら保存的治療が行われる．四肢の長管骨に発生した場合，切除による機能障害の可能性から切除をためらうこともあるが，肋骨に発生した場合は，切除による機能障害は大きなものではない．増大してからの切除は侵襲が大きくなり，また，悪性転化の可能性もある．よって，肋骨発生の骨線維性異形成は積極的に切除を行うべき...
