The current report describes a patient with pulmonary embolism, treated unsuccessfully with heparin. Transthoracic echocardiography revealed free-floating right heart thrombus. Migrating deep vein thrombus to the right heart was suspected. Transesophageal echocardiography confirmed origin of the thrombus in the inferior cava vein. Mortality rate of mobile right heart thrombus is over 40%, there...
Results This patient had serial preoperative examinations including transthoracic and transesophageal echocardiographies, and cardiac catheterization before undergoing cardiac surgery for reconstruction of right ventricular outflow tract and repair of aortic and tricuspid valves. She had an uneventful postoperative course and remained asymptomatic afterward. Patients of similar conditions had b...
We describe the case of a 61 year-old female patient admitted to the hospital with acute lateral myocardial infarction, in pulmonary edema, who evolved to cardiogenic shock in the first hours. Transesophageal echodoppler-cardiogram and nuclear magnetic resonance imaging showed the diagnosis of a left ventricular pseudoaneurysm. Surgical repair was successfully undertaken.
We would like to express our enthusiasm with the repercussion of the article “Change in Surgical Conduct Motivated by Intraoperative Transesophageal Echocardiogram”, since the two letters published in the last issue of the journal mentioned it. Regarding the brilliant explanation on the estimated systolic blood pressure of the pulmonary artery in cases of right ventricular outlet (RVO) obstruct...
We describe a case of acute unilateral left pulmonary edema occuring in an old man one year after mitral valve replacement. Transesophageal echocardiography identified a paravalvular leakage with severe mitral regurgitant jet directed to both left pulmonary veins. A prompt surgical closure of the leakage by pericardial patch was decided with rapid clinical and radiological improvement.
Scimitar syndrome is a rare association of congenital cardiopulmonary anomalies characterized by partial anomalous pulmonary venous return, in which an abnormal right pulmonary vein drains into the inferior vena cava. This case exemplifies the role of transesophageal echocardiography in perioperative management and surgical decision-making.
An anomalous right coronary artery arising from the left sinus of Valsalva is a rare but potentially lethal abnormality. We present a case report and literature review of this anomaly as well as its surgical management in the face of unobstructed distal coronary arteries. Furthermore we report the use of intraoperative transesophageal stress echocardiography to evaluate adequacy of graft flow.
Arguments Against Left Atrial Appendage Occlusion for Stroke Prevention To the Editor: With great interest we read the article by Onalan et al about left atrial appendage (LAA) exclusion for stroke prevention in nonrheumatic atrial fibrillation (AF).1 We share the author’s concerns about the hazards of this procedure and would like to emphasize additional arguments. There is no evidence that th...
