根尖性歯周炎は根管内病原体による炎症性疾患とされており,その治療手順の一つである根管洗浄は清掃器具の届かない領域に残存する根管内病原体の除去に重要である.Er:YAGレーザー等を用いた根管洗浄法(laser-activated irrigation: LAI)は衝撃波,キャビテーションおよび高速水流を生じる優れた根管清掃法として報告されているが,清掃の至適条件の不明確さや根尖孔外への洗浄剤の溢出への懸念がある.本総説では,Er:YAGレーザーによるLAIの清掃効果と安全性について過去の報告を元に概説するとともに我々の研究を紹介する.
