Common bile duct disruption from blunt trauma is very rare. Management, diagnosis and therapy by a non-specialist surgeon can be difficult. We describe a bile duct injury after a motor vehicle crash in a young male, treated with cholecystojejunostomy at his third laparotomy. We also briefly review some diagnostic aspects and therapeutic options from the literature.
BACKGROUND According to the literature, benign bile duct tumors are exceedingly uncommon. To the best of our knowledge, we report the largest extrahepatic bile duct villous adenoma described in the literature. CASE PRESENTATION We present a case of a 77-year-old Caucasian woman with obstructive jaundice. Laboratory tests revealed that she had elevated bilirubin and liver enzyme levels. A comp...
Bile duct injuries in laparoscopic cholecystectomy are briefly reviewed. The Norwegian National Cholecystectomy Registry was started on April 1, 1993, to collect data from most Norwegian surgical departments. in the period April 1, 1993-May 31, 1995, common bile duct (CBD) injuries necessitating treatment were reported in 0.61% of 2,612 laparoscopic cholecystectomies and in 0.74% of 674 open ch...
Background: the presentation of common bile duct disease, value of investigations and treatment outcome in elderly patients undergoing endoscopic retrograde cholangiopancreatography (ERCP) were assessed. Methodology: the clinical presentation, liver function tests, full blood counts, abdominal ultrasound and ERCP results were assessed retrospectively in 101 patients (59 women, 42 men; mean age ...
BACKGROUND AND STUDY AIMS Endoscopic removal of asymptomatic common bile duct stones (CBDS) is generally recommended. Although many reports have described the risk of complications in endoscopic retrograde cholangiopancreatography (ERCP), no studies have addressed this problem in the context of asymptomatic CBDS. This study examines the risk of complications arising in ERCP for asymptomatic CB...
Bile is normally sterile. Bactibilia occurs when bile flow is impaired and bacteria gain access to the biliary duct by the papilla or portal circulation. Bile duct obstruction leads sequentially to increase in intra biliary pressure, infection of bile, cholangiovenous and cholangiolymphatic reflux leading to bacteremia [1-3]. The bacteria described commonly in biliary tract infections include E...
Most patients with unresectable, malignant, obstructive, cholangiocarcinoma are candidates for palliation. Biliary drainage by endoscopic interventions (ERCP), with implantation of self-expandable metallic stents (SEMSs), plays a major role in improving liver function and managing or avoiding cholangitis [1]. We present two cases (a 78-year-old man and a 65-year-old woman) with advanced, unrese...
Cystic dilatation of the common bile duct constitutes a rare congenital malformation prevalent in people of Asian origin with a female predominance. Its classification follows that of Todani. The commonest form, found in 80% of cases, is type I. Common bile duct cysts are often diagnosed in 2 out of 3 cases, during childhood with adult forms often rare. We hereby report the case of a cystic dil...
An 83-year-old woman was admitted because of colicky pain in the abdominal right upper quadrant and a cholestatic pattern in liver chemistry. Transcutaneous abdominal ultrasound showed a huge dilation of the extrahepatic bile duct. A distal common bile duct stone was also observed. An endoscopic retrograde cholangio-pancreatography (ERCP) was performed. Vater’s papilla was located in the right ...
Intraductal papillary neoplasm of the bile duct (IPNB) represents biliary papillary tumors mainly growing and is considered to be of relatively low-grade malignancy. Here we report a case of IPNB in whom the poorly differentiated component deeply infiltrated the bile duct wall. A 77-year-old male had an invasive carcinoma of the bile duct 3 cm in size. He underwent right hemihepatectomy with co...
