症例は85歳，女性．S状結腸癌（pStage IIa）に対して腹腔鏡下S状結腸切除術，DST再建を施行した．術後4年目の外来フォロー中に右側方リンパ節転移を伴う局所進行直腸癌を認めたため，腹腔鏡下直腸切断術，右骨盤神経叢，右内腸骨動静脈合併切除を伴う右側方リンパ節郭清（D1LD1（Rt-2，Lt-0））を施行した．病理結果はpT3N3M0 pStage IIIcであり，前回吻合部より口側の腸間膜リンパ節（#241）に転移を二つ認めた．異時性大腸癌において吻合部を越えてリンパ節転移をきたすという報告は乏しく，郭清範囲についても定まった基準はない．今回われわれは，過去の手術でリンパ管の交通が途絶えたにもかかわらず，吻合部を越えて腸間膜リンパ節転移をきたした症例を経験したので，文献的考察を加えて報告する．
