Forty failed hinged arthroplasties of the knee were revised by the insertion of another hinged implant. In 14 cases the prosthesis used at the revision operation was similar to the primary implant; in 26, a hinge with an elongated femoral stem was used, usually replacing part of the femoral shaft. In seven of these knees an elongated tibial stem was also required, though the tibial shaft was re...

INTRODUCTION Femoral centralizers in total hip arthroplasty (THA) are designed to improve the neutral implant position and ensure a homogeneous cement mantle without implant-bone impingement. To date there are no data about the cement mantle configuration and implant position after malinsertion, as seen in mini-open approaches or adipose patients with a limited view. The present biomechanical s...

We report an 82-year-old woman who underwent fixation with a long-spanning cable-plate for a bisphosphonate-induced Vancouver B1 periprosthetic femoral fracture. Non-union and breakage of the plate occurred at 16 months and necessitated revision surgery using a long-stem femoral prosthesis augmented with a cable-plate construct. Bone union was achieved eventually after 10 months.

Ten uncemented total hip replacements were performed in 1975 using an implant in which the cobalt-chrome femoral stem was coated to give a porous surface. In all but one case a high-density polyethylene head was used. The radiological changes in the upper femoral shafts were assessed between three and nine years later. Seven showed extensive stress-relieving changes, loss of calcar, stress frac...

PURPOSE To report the radiological and clinical results of revision total hip arthroplasty (THA) with femoral impaction bone grafting (IBG). METHODS 4 men and 9 women (15 hips) aged 38 to 84 years underwent revision THA with femoral IBG using Elite plus stems for aseptic loosening or infection. RESULTS The mean follow-up duration was 7.7 years. The Harris hip score improved from 61.7 to 83....

Peri-prosthetic femoral fracture after total hip replacement (THR) is associated with a poor outcome and high mortality. However, little is known about its long-term incidence after uncemented THR. We retrospectively reviewed a consecutive series of 326 patients (354 hips) who had received a CLS Spotorno replacement with an uncemented, straight, collarless tapered titanium stem between January ...

An artificial joint (prosthesis) helps reduce pain and improve function. Also called total hip arthroplasty, replacement surgery might be an option if interferes with daily activities nonsurgical treatments haven't helped or are no longer effective. We review the different materials, constructions, surfaces used in development of femoral stems, order to solve avoid various problems associated t...

PURPOSE To evaluate risk factors for early dislocation after primary total hip arthroplasty (THA). METHODS Records of 175 cases with dislocation during hospitalisation after THA and 651 controls without dislocation were reviewed. Cases and controls were matched for age, gender, body mass index classification, primary diagnosis, cup design, hospital, and year of intervention. Version and incli...

Custom femoral components have been developed for total hip arthroplasty to maximize cortical form-fit and thereby to reduce the problems of stress shielding and aseptic loosening. Limited information is available about how much endosteal cortical contact can actually be achieved with these expensive implants. The aim of this study was therefore to verify the exact cortical contact of a custom ...
