Sporadic Medullary Carcinoma of the Thyroid is a relatively uncommon entity and at the time of diagnosis, most already present loco-regional metastasis. Therapy should be aggressive to reduce recurrence and mortality. The intravenous infusion of the octapeptide of cholecystokinin inhibited the response of the lower oesophageal sphincter to graded doses of pentagastrin in normal men. The possibility that the effects of these peptides on the lower oesophageal sphincter are only pharmacological and not physiological is discussed.
