We present the case of a patient who developed periorbital ecchymosis (panda sign) following an exacerbation of asthma with severe coughing. There was no history of cranial trauma. Investigations for a structural or haematological cause were negative. The panda sign is typically seen following a base of skull fracture, but rarely reported due to severe coughing.

Clinical presentation of carotid artery dissection (CAD) is variable, including acute or subacute headache, cervical pain, or focal neurologic deficit that occurs spontaneously, after a high-speed neck manipulation, or trauma. Isolated lower cranial nerve palsy in carotid dissection is uncommon.
A case of internal carotid artery dissection presenting as delayed right hemilingual paresis after blunt craniocervical trauma is presented. Diagnosis is discussed with emphasis on MR and MR angiographic findings. Mechanisms of injury and lower cranial nerve palsy are also briefly discussed.
