We present a very rare case of intracranial xanthoma with bilateral temporal bone involvement. The lesion caused destruction and remodelling in mastoid air cells. On the right side, cerebellar compression was obvious. The patient was operated on the symptomatic side and the lesion was totally removed.

We report a patient with relapsing-remitting multiple sclerosis (RRMS), who developed bilateral severe tongue weakness due to the anterior opercular syndrome. This was caused by a recent inflammatory demyelinating lesion in the right perisylvian juxtacortical region, superimposed on a pre-existing left perisylvian lesion, which had previously caused temporary isolated right tongue weakness.

CT showed a serpiginous calcified interhemispheric fissure lesion with contrast enhancement.
