術前に高カルシウム（Ca）血症が明らかではなかった副甲状腺癌症例を経験したので報告する。症例は73歳女性。透析導入期の加療目的に当院腎臓内科を紹介受診し，経過中にintact PTH高値と左副甲状腺腫を指摘され手術目的で当科紹介となった。既にシナカルセトが導入されており高Ca血症を認めなかったが，左上皮小体摘出術を実施し，副甲状腺癌の病理診断であった。従来，高Ca血症は副甲状腺癌を疑う所見のひとつであるが，本症例では高Ca血症の悪化を来す前にCa受容体作動薬による内服治療が開始されており顕在化しなかった。われわれ頭頸部外科医も内科的治療への理解を深める必要がある。
