This article will appear in the January 1, 2013, issue of AJHP. Accountable care organizations (ACOs) are groups of health care providers (e.g., physicians, hospitals, health systems) that are jointly held responsible for improving the quality of care and reducing costs (or the rate of growth in costs) across the health care continuum, including the ambulatory care, inpatient, and long-term-car...

Since early 1985, four social health maintenance organizations have delivered integrated health and long-term care services to Medicare beneficiaries under congressionally mandated waivers that included shared public-program risk for losses. Three of four sites had substantial losses in the first 3 years, primarily because of slow enrollment and resultant high marketing and administrative costs...

This paper describes the network architecture of an Asiawide satellite Internet that considers the situations in developing regions. The design considerations for the architecture are costs, effective use of satellite bandwidth, scalability, and routing strategy when combined with terrestrial links. The architecture includes using one-way shared satellite links to reduce costs, IP multicast to ...

In this paper, we propose a model for medical costs recorded at regular time intervals, e.g. every month, as repeated measures in the presence of a terminating event, such as death. Prior models have related monthly medical costs to time since entry, with extra costs at the final observations at the time of death. Our joint model for monthly medical costs and survival time incorporates two impo...

This paper deals with the minimization of inter-task and inter-processor communication costs in the context of one SQL query mapping onto a sharednothing architecture, as far as the parallel decisional query processing is concerned. After setting both the models of the application handled and the target multiprocessor architecture, a study aimed at minimizing both inter-processor transfer times...

With the implementation of the Affordable Care Act (ACA), access to insurance and coverage of preventive care services has been expanded. By removing the barrier of shared costs for preventive care, it is expected that an increase in utilization of preventive care services will reduce the cost of chronic diseases. Early detection and treatment is anticipated to be less costly than treatment at ...

We analyze the costs, and describe the implementation, of three hashed-based join algorithms for a general-purpose shared-memory multiprocessor. The three algorithms considered are the Hashed Loops, GRACE and Hybrid algorithms. We also describe the results of a set of experiments which validate the cost models presented and demonstrate the relative performance of the three algorithms.

Motivated by applications in freight handling and open-pit mining, Rhys, Balinski and Picard studied the problems of selection and closure in papers published in Management Science in 1970 and 1976. They identified efficient algorithms based on linear programming and maximum-flow/minimum-cut procedures to solve these problems. This research has had major impact well beyond the initial applicati...

An issue of considerable importance, both from a practical organizational standpoint and from a costs research perspectives, involves the allocation of /xed resources or costs across a set of competing entities in an equitable manner. Cook and Kress (Eur. J. Oper. Res. 119 (1999) 652) propose a data envelopment analysis (DEA) approach to obtain a theoretical framework for such cost allocation p...
