KEY CLINICAL MESSAGE Placenta percreta (with bladder invasion) is a rare obstetric condition with the risk of massive intraoperative hemorrhage. In these cases, the combination of a transverse uterine fundal incision and retrograde cesarean hysterectomy could be useful to minimize maternal hemorrhage and avoid severe bladder injury.
Placenta percreta is an extremely rare and the most severe form of placental invasion, that is associated with severe maternal morbidity and mortality. We report a case of nulliparous woman who underwent 10 cycles of in vitro fertilization (IVF) without any known risk factors. Placentation disorders are the result of impaired embedding of the placenta in the endometrium. The prevalence of these disorders is estimated to be around 0.3 %. A history of previous prior uterine surgery (especially cesarean section and curettage) is the most common risk factor. Impaired placentation is differentiated into deep placental attachment; marginal, partial and total placenta previ...
I NVASION of the uterine surface by the chorionic villi is Iimited by protective eIements in the decidua basalis. Wislockr has shown the presence of a mucopoIysaccharide in the decidua that serves to limit trophobIastic erosion of maternal tissue. Microscopic sections from most cases of placenta accreta show an absence of the decidua basaIis and atrophy of the decidua Vera. The degree of invasi...
