We report 2 cases of congenital pericardial defect with ruptured acute type A aortic dissection. Case 1: An 83-year old man presented with sudden chest and back pain, and computed tomography (CT) showed acute aortic dissection with left pleural massive effusion. Because of his unstable haemodynamic condition with low blood pressure, an emergency operation was performed. We observed small amount...

Classically, ascending aortic dissection (AD) presents as sudden, severe chest pain that is tearing type and radiates to the back. Herein, we present a rare case of severe aortic regurgitation with silent ascending AD, which had no chest pain symptoms. The aortic valve apparatus probably masked this AD; therefore, it was not detected by echocardiography and during the surgery.
