Trauma to the spinal column may cause a variety of clinical syndromes depending on the type and severity of the impact and bony displacement as well as secondary insults such as hemorrhage, ischemia, and edema. Complete spinal cord injury results in a transverse myelopathy, with total loss of spinal function below the level of the lesion. This insult is caused by either anatomic disruption of t...

The cytokine erythropoietin (EPO) possesses potent neuroprotective activity against a variety of potential brain injuries, including transient ischemia and reperfusion. It is currently unknown whether EPO will also ameliorate spinal cord injury. Immunocytochemistry performed using human spinal cord sections showed abundant EPO receptor immunoreactivity of capillaries, especially in white matter...

Thoracoabdominal aneurysm surgery is associated with a high incidence of morbidity and mortality. Spinal cord ischemia and the risks of paraparesis or paraplegia remain devastating complications. The mechanisms of spinal cord injury involve both acute ischemic injury and delayed reperfusion injury. Blood flow to the spinal cord frequently arises in the segment of the aorta requiring aortic cros...

BACKGROUND Paraplegia remains a devastating complication of thoracic aortic surgery. The mechanism of the antecedent spinal cord ischemia and reperfusion injury (IR) remains poorly described. IR involves 2 injuries, an initial ischemic insult and subsequent inflammatory amplification of the injury. This mechanism is consistent with the clinical phenomenon of delayed onset paraplegia. This study...

Three patients presented with hand wasting and weakness secondary to mid-cervical spinal cord compression. This was due to cervical spondylosis in two patients and a meningioma in one case. This phenomenon is probably similar to that seen with foramen magnum lesions and may be due to spinal cord ischemia distal to the compression, secondary to venous stasis.

Thoracoabdominal aneurysm surgery carries an approximate 10% risk of intraoperative paraplegia. Abrupt cord ischemia and the confounding effects of systemic alterations and limb or cerebral ischemia challenges neurophysiologic spinal cord monitoring. This investigation sought a rapid differential monitoring approach to predict or help prevent paraplegia. Thirty-one patients were monitored with ...

The authors present a review of spinal cord blood supply, discussing the anatomy of the vascular system and physiological aspects of blood flow regulation in normal and injured spinal cords. Unique anatomical functional properties of vessels and blood supply determine the susceptibility of the spinal cord to damage, especially ischemia. Spinal cord injury (SCI), for example, complicating thorac...

Studies on the experimental spinal contusion injury in animals confirm that posttraumatic ischemia contributes to central cystic necrosis or fibrosis occurring at the level of the spinal cord lesion. Hyperbaric oxygen (HBO) modifies the degree and extent of the pathology in the spinal cord of the experimental animals. HBO has been used for 45 patients with recent spinal cord injuries. The exten...

Introduction: Ischemia plays an important role in the development of pathologic in various neuropathies, and reperfusion improves the pathological and physiological abnormalities of ischemic nerves. In this study, the effect of shortreperfusion time after short time ischemic on spinal cord of rat was investigated. Methods: Eighteen rats (Sprague-Dawley, 250-350 g), were divided into 3 groups: g...

BACKGROUND AND PURPOSE Hemodilution using modified hemoglobin solutions may reduce ischemic central nervous system injury. Purified diaspirin cross-linked hemoglobin (DCLHb) is a cell-free hemoglobin that is intramolecularly cross-linked between the two alpha subunits, resulting in enhanced oxygen offloading to tissues and increased half-life. In the present experiments, we evaluated the abilit...
