AIM The benefit of preventive percutaneous coronary intervention (PCI) in ST elevation myocardial infarction (STEMI) has been shown in randomised trials. However, all the randomised trials are underpowered to detect benefit in cardiac death. We aim to systematically review evidence on the cardiac mortality benefit of preventive PCI in patients presenting with acute STEMI in randomised patient p...
BACKGROUND Fisher syndrome is one of the regional variants of Guillain-Barré syndrome, characterised by impairment of eye movements (ophthalmoplegia), incoordination (ataxia) and loss of tendon reflexes (areflexia). It can occur in more limited forms, and may overlap with Guillain-Barré syndrome. A further variant is associated with upper motor neuron signs and disturbance of consciousness (Bic...

BACKGROUND Cluster randomised trials can be susceptible to a range of methodological problems. These problems are not commonly recognised by many researchers. In this paper we discuss the issues that can lead to bias in cluster trials. METHODS We used a sample of cluster randomised trials from a recent review and from a systematic review of hip protectors. We compared the mean age of particip...
What is the best available evidence for treatment of first metatarsophalangeal joint osteoarthritis?
Methods Randomised controlled trials, quasi-randomised trials, or controlled clinical trials that assessed treatment outcomes for first MTPJ OA were searched for across several electronic databases (to the 14th January 2010) (CENTRAL; MEDLINE; EMBASE; CINAHL; and PEDro). Participants of any age or gender with OA of the first MTPJ defined either radiographically or clinically were included in th...
In this article, the principle of randomised trials are first described and then prostate cancer screening trials published to date are evaluated based on these principles. A summary of the randomised prostate cancer screening is provided. The conclusion that can be made from the results of the screening trials, as well as limitations of the evidence and open questions are outlined in the end.
Randomised controlled trials were reviewed to evaluate the evidence of the effectiveness of extracorporeal shock wave therapy in the management of tennis elbow. Seven relevant trials were found, which had satisfactory methodology but conflicting results. Further research with well designed randomised control trials is needed to establish the absolute and relative effectiveness of this intervent...

DESIGN eight relevant citation databases were searched up to January 2013 for all randomised trials, and ...
