Carcinoma ex pleomorphic adenoma is an exceedingly rare neoplasm of the minor salivary gland. Prognostic parameters are recurrence, capsular invasion, and metastasis. We present a case of carcinoma ex pleomorphic adenoma with pulmonary metastasis to emphasize that patients treated for this condition should be investigated for distant metastasis.

A 67-year-old patient presented for persistent cough. Computed tomography showed right lower lung opacity associated with mediastinal adenopathy. On suspicion of metastatic pulmonary neoplasm, the patient was submitted to right lower lobectomy with lymphadenectomy. Postoperative histopathology led to the diagnosis of multicentric Castleman's disease.

Pulmonary tumor thrombotic microangiopathy is a rare but serious malignancy-related respiratory complication. The most common causative neoplasm is gastric adenocarcinoma. We report a case caused by metastatic prostate adenocarcinoma, diagnosed postmortem in a 58-year-old male. To our knowledge, this is the second reported case from metastatic prostate adenocarcinoma.

Alveolar adenoma, a rare benign pulmonary neoplasm, usually presents as asymptomatic. Since first described in 1986, no more than 35 cases have been reported in the English medical literature. Here we report a case of 48-year-old woman who suffered from this tumor, the patient is doing well 4 years after thoracoscopic lobectomy.
