【背景】TKA術後の屈曲制限に対して早期のマニプレーションは有効であるが，その際の疼痛が課題である．TKA術後鎮痛には硬膜外麻酔（以下EA）や内転筋管ブロック（以下ACB）が有効とされる．今回，当院で行った術後マニプレーション時の鎮痛，可動域改善目的としてEAとACBを比較・検討した．【対象と方法】2020年4月から2022年1月まで当院で初回TKA施行された患者87名を対象に術後7日目にEA下，8日目にACB下にマニプレーションを実施し，実施前後の可動域，疼痛をそれぞれ比較した．【結果】EA実施後に可動域の改善を認め，ACB実施後も可動域の改善を認めた．EAとACB間では実施後における可動域改善には有意差を認めなかった．NRSによる評価ではEA実施後には疼痛改善を認めたが，ACB実施後の疼痛改善を認めなかった．EAとACBの比較では患者満足度はほぼ同数（28対25）だった．ACBは...
