Abstract:

Background and Aims: Cells and secreted molecules by the innate immune system are the essential factors in the pathogenesis and determining the severity of inflammation in COVID-19 patients. Severe inflammation results from increased activity of neutrophils, macrophages, and other cells with their products. Inflammatory cytokines such as tumor necrosis factor-α (TNF-α) increases the severity and pathogenesis of the disease caused by the virus. Phagocytes are armed with inducible nitric oxide synthase (iNOS), that upon stimulation by proinflammatory cytokines augment an immune response against pathogens. Materials and Methods: Two groups of patients were included with COVID-19 infection from the intensive care unit (ICU, n=52) and (non-ICU-care, n=54). Blood samples were collected to measure cells and serum parameters, including lymphocytes, neutrophils, platelet counts, accompanied with C-reactive protein, lactate dehydrogenase, TNF-α and iNOS levels. Results: In the ICU group, increased white blood cells (p=0.048), decreased lymphocytes (p=0.0007), increased neutrophils (p=0.001), decreased platelets, increase serum levels for lactate dehydrogenase (p =0.0001), c-reactive protein (p=0.003), TNF-α (p=0.018), and iNOS (p=0.008) were statistically obtained. Positive correlations were calculated between TNF-α and iNOS (r=0.65, p=0.0002) and with c-reactive protein (r=0.52, p=0.003) and with lactate dehydrogenase (r=0.68, p=0.0001). Conclusion: Inflammation due to macrophages and neutrophils activity in COVID-19 patients and increased mediators correlate with disease progression. It seems that control of the cell activity and their inflammatory cytokines would be considered for therapeutic goals. Changing the polarization of inflammatory macrophages to anti-inflammatory macrophages with therapeutic applications could prevent the severity of the provocative course of the disease.

The risk factors related to liver injury in non-Intensive Care Unit (non-ICU) admitted patients infected with COVID-19

Background: Liver is one of the target organs for COVID-19 infection. The liver damage in critically ill patients was investigated in previous studies, but there is no available data about liver injury in mild to moderate form of infection with COVID019. In this study, we estimated the prognostic factors related to liver injury in NON-ICU admitted patients infected with COVID-19. Methods: in t...

Calculation of Direct Medical Costs and Indirect Costs in Patients with Covid-19 Hospitalized in the Intensive Care Unit in Golestan Province

Abstract Background: Healthcare policymakers and planners use the economic burden of the disease to determine the total cost of an outcome, illness, or accident imposed on the country. The aim of this study was to calculate the direct and indirect medical costs in Covid-19 patients with critical conditions hospitalized in the intensive care unit in Golestan province. Methods: This descriptive...

Death due to resistance to enterococci in the intensive care unit: case report

Background & Aim: Hospital infections are one of the major problems in intensive care units and in many cases increases the mortality rate. Case report: A 68-year-old man is being treated after cervical vertebra fracture and fracture and is treated with linezolid following antibiotic resistance. Following the non-observance of the isolated contact points, this resistance was transferred to the ...

Evaluation of Epidemiological, Paraclinical, and Imaging Findings of Patients With ‎Covid-19 Hospitalized in Intensive Care Unit of Rasht Hospitals in March 2020

Background: Considering the high prevalence of Coronavirus Disease 2019 (COVID-19) and the medical resource constraints, a better understanding of the characteristics of the disease and identifying the factors indicating the severity of the disease‎ can help decide on the allocation of important and limited resources. The current study aimed to investigate the demographic, clinical, laboratory ...

The prevalence of Mycoplasma pneumoniae infection in patients with ventilator-associated pneumonia hospitalized in intensive care unit

Background: Ventilator-associated pneumonia (VAP) is a type of hospital acquired pneumonia with the mortality rate between 27% and 76% that develops more than 48–72 h after endotracheal intubation. Possible causes leading to this infection can be Mycoplasma pneumoniae. The objective of this study was to determine the presence of Mycoplasma pneumoniae in bronchoalveolar samples of patients with ...
