Recognition of left atrial aneurysm by fetal echocardiography.

چکیده
A fetus was noted to have a mediastinal mass on prenatal ultrasound. Fetal echocardiography at 26 weeks of gestation demonstrated a large cystic structure adjacent to the left atrium and left ventricle (Figure 1). The baby was born full term. Echocardiography revealed a large, thin-walled aneurysm (333 cm) lateral to the left atrium and communicating with it through an orifice posterior to the base of the left atrial appendage. As determined by MRI, this aneurysm extended from the transverse aortic arch to the cardiac apex (Figure 2). Angiograms were obtained with the catheter tip positioned within the aneurysm (Figure 3). None of these imaging modalities revealed thrombus within the aneurysm. At 29 days of age, the aneurysm was approached surgically through a median sternotomy under cardiopulmonary bypass; it appeared thin-walled and translucent and extended along the lateral aspect of the left ventricle (Figure 4). When the aneurysm was opened, it was evident that a portion of its medial wall consisted of left ventricular epicardium (Figure 5). The aneurysm was resected, and its left atrial orifice was closed using autologous pericardium. Follow-up echocardiography revealed no recurrent aneurysm. Most patients with left atrial aneurysms present with arrhythmia or abnormal cardiac silhouette on chest radiography.1 This is the first case to be recognized by fetal echocardiography.

Unusual consequence of a fetal atrial septal aneurysm
We describe the case of a significant fetal atrial septal aneurysm causing left ventricular inflow obstruction. Serial fetal echocardiograms demonstrated potential left heart hypoplasia. The fetal cardiologist guided the perinatal team to electively deliver the infant early and modify the course of developing hypoplastic left heart.
متن کاملRight Atrial Appendage Aneurysm in a Newborn Diagnosed with Fetal Echocardiography
Right atrial appendage aneurysm is a very rare condition which can be asymptomatic or can cause arrhythmia or life-threatening thromboembolism. We report a case of newborn with right atrial appendage aneurysm who was diagnosed with fetal echocardiography. Anticoagulant therapy was applied to prevent thromboembolism and he is still going on follow-up without any complaint.
متن کاملCongenital aneurysm of both left ventricle and left atrium
This is a case of both congenital left ventricular (LV) free wall submitral aneurysm and left atrial appendage aneurysm with 6 years of clinical follow-up. Each lesion is a rare entity, and to the best of our knowledge, this is the first case in medical literature of both lesions occurring in the same patient, raising the likelihood of a common etiology. The workup was initiated in the third tr...
متن کاملCongenital aneurysms of the left atrium: recognition by cross-sectional echocardiography.
The two-dimensional echocardiographic features of three patients with congenital aneurysms of the left atrium are described. The aneurysm arose from the left atrial appendage in two patients and from the posterior left atrial wall in one. The aneurysms were characterized by their origin from an otherwise normal left atrium, a well-defined neck, their position within the pericardial space, and d...
متن کاملLeft atrial appendage aneurysm: correlation of noninvasive with clinical and surgical findings: report of a case.
Congenital left atrial appendage aneurysm is rarely diagnosed on the basis of an abnormal cardiac silhouette. Patients with a left atrial appendage aneurysm often present with symptoms of systemic emboli or supraventricular arrhythmias. A patient with left atrial appendage aneurysm was diagnosed by correlation of two noninvasive techniques echocardiography and radionuclide scintiscanning. Angio...
