Home mechanical ventilation.

چکیده
Mechanical ventilation originated as an in-hospital service but there is now an increasing demand for provision at home [1]. This is a consequence of social pressures, a better understanding of the significance of nocturnal hypoventilation in the pathogencsis of respiratory failure, and an awareness of respiratory muscle fatigue. It is important to recognize that there are two quite different circumstances in which respiratory support may be needed: 1) the patient with a complete inability to breathe, for example after high cervical cord transection; 2) the patient unable to sustain adequate spontaneous ventilation indefinitely, in whom assistance during sleep is all that is required.

Ethical challenges in home mechanical ventilation: A secondary analysis
The aim of this study was to explore the ethical challenges in home mechanical ventilation based on a secondary analysis of qualitative empirical data. The data included perceptions of healthcare professionals in hospitals and community health services and family members of children and adults using home mechanical ventilation. The findings show that a number of ethical challenges, or dilemmas,...
متن کاملLong-term home mechanical ventilation in the United States.
Patients requiring prolonged mechanical ventilation are rapidly increasing in number. Improved ICU care has resulted in many patients surviving acute respiratory failure to require prolonged mechanical ventilation during convalescence. Also, mechanical ventilation is increasingly used as a therapeutic option for patients with symptomatic chronic hypoventilation, with an increased effort to pre...
متن کاملNoninvasive home ventilation for chronic obstructive pulmonary disease: indications, utility and outcome.
PURPOSE OF REVIEW Despite its widespread use, the role of noninvasive home mechanical ventilation for the management of severe chronic obstructive pulmonary disease and chronic hypercapnic respiratory failure is still controversial. The majority of randomized controlled trials show methodological weaknesses, including issues of patient selection, insufficient pressure support and poor adherence...
متن کاملProvision of home mechanical ventilation and sleep services for England survey.
The Department of Health is promoting the generation of specialist networks to manage long term ventilatory weaning and domiciliary non-invasive ventilation patients. Currently the availability of these services in England is not known. We performed a short survey to establish the prevalence of sleep and ventilation diagnostic and treatment services. The survey focussed on diagnostic services a...
متن کاملWho benefits from home mechanical ventilation?
chronic respiratory failure (CRF) in the home has been one of the major advances of respiratory medicine. The delivery of ventilatory support has progressed from negative pressure devices, introduced as a consequence of the poliomyelitis epidemics in the 1940s and 1950s, through ventilation via tracheostomy to non-invasive ventilation (NIV) via face or nasal masks. NIV has expanded from its ini...

Definition and goals ofhome mechanical ventilation Born during the polio epidemics of 50 years ago, home mechanical ventilation has evolved towards two distinct concepts-a life support system for patients without respiratory independence (high cervical spinal cord injury, end stage neuromuscular disease, or chronic obstructive pulmonary disease), and elective therapy for patients with progressi...
