چکیده

Sir,—Brock-Utne and Jaffe recommend the sitting position for fibreoptic intubation in the presence of a full stomach [1]. I would endorse this for most patients undergoing fibreoptic intubation, as there are several advantages in addition to those mentioned in their letter. The sitting position is more natural for the patient and allows face-to-face communication with the anaesthetist. This makes for a more relaxed patient with less need for sedation and thus a truly "awake" intubation. Any blood or secretions drain from the pharynx or can be swallowed more easily, making for a clearer view. It is also a more physiological position for the patient with obesity or respiratory or cardiac disease. The only objection is that the view is "the wrong way round", but with a little practice and imagination this is not a problem. Originally, I adopted the use of the sitting position for fibreoptic intubation because I copied the technique of physicians when performing fibreoptic bronchoscopy. On a recent visit to the Department of Anesthesiology at the University of Michigan, I was encouraged to find that their "Difficult Airway Clinic" endorsed this practice [2]. Presumbly, anaesthetists place their patients in the supine position from habit.

Total i.v. anaesthesia with propofol-alfentanil or propofol-fentanyl.

In combination with propofol, alfentanil was compared with fentanyl for total i.v. anaesthesia in 29 women (ASA classes I, II) admitted for elective hysterectomy. Infusion rates of propofol and fentanyl were determined from the literature and from pilot studies, while alfentanil was given according to a computer program. Dosage was: propofol, bolus 1.5 mg kg-1, infusion 9 mg kg-1 h-1 for 10 min...

Prevention of propofol-induced pain in children: combination of alfentanil and lidocaine vs alfentanil or lidocaine alone.

BACKGROUND Pain from a propofol injection is a common side-effect in paediatric patients. This prospective, randomized, double-blind study evaluated the efficacy of a combined pretreatment of alfentanil with lidocaine on the incidence and severity of propofol injection pain in children. METHODS After obtaining parental consent, 120 paediatric patients were allocated randomly into one of the t...

Pharmacokinetics of an infusion of propofol during cardiac surgery.

We have measured whole blood concentrations and pharmacokinetics of propofol administered as a constant rate infusion during cardiac surgery. Ten patients undergoing elective cardiac surgery involving cardiopulmonary bypass (seven myocardial revascularization and three aortic valve surgery) received a continuous infusion of propofol 4 mg kg-1 h-1 to supplement alfentanil analgesia. Whole blood ...

Effect of methohexitone and propofol with or without alfentanil on seizure duration and recovery in electroconvulsive therapy.

We have studied the effects of methohexitone and propofol with and without alfentanil on seizure duration and recovery in this observer-blinded, prospective, randomized, crossover study involving 24 patients undergoing electroconvulsive therapy (ECT). Each patient had four treatment sessions, and received the following four i.v. regimens in random order: methohexitone 0.75 mg kg-1, methohexiton...
