چکیده

We report a six year-old asthmatic male who developed transient motor tics, possibly secondary to his use of fluticasone propionate (fluticasone). At this particular clinic visit, the mother reported that the patient had begun using fluticasone 50 mcg two puffs b.i.d. for the last four months. The motor tics began to appear about two months after he began the fluticasone, and continued throughout that time period. His fluticasone was replaced with ciclesonide (100 mcg one puff daily), and within eight weeks, his motor tics resolved. Furthermore, we report a ten year-old male with asthma and Tourette’s syndrome (TS) whose tics worsened five days after the introduction of fluticasone 125 mcg two puffs b.i.d. When fluticasone was discontinued, his tics returned to baseline. Due to asthma symptoms, he was prescribed fluticasone again, but at a dose of 50 mcg two puffs b.i.d. Once again a similar exacerbation of his tics was noted, which again resolved after discontinuation of fluticasone. The purpose of this letter is to speculate a link between fluticasone and tics in the pediatric asthmatic population based on these cases and a literature review.

Basal cortisol in asthmatics on two different doses of fluticasone propionate.

OBJECTIVES To evaluate basal plasma cortisol in persistent asthmatics on inhaled fluticasone propionate 200 mcg/day and 300 mcg/day. METHODS Asthma diagnosis and classification was based on Global Initiative for Asthma recommendations. Patients aged 11 years old or less received fluticasone propionate 200 mcg/day and those older than 11 years received 300 mcg/day. After 10 weeks of treatment,...

Comparison of fluticasone propionate and beclomethasone dipropionate on direct and indirect measurements of bronchial hyperresponsiveness in patients with stable asthma.

BACKGROUND Fluticasone propionate is a new inhaled corticosteroid with a 2:1 efficacy ratio compared with beclomethasone dipropionate with regard to lung function and symptom scores, without increased systemic activity. The aim of this study was to investigate whether this was also the case for bronchial hyperresponsiveness, assessed by both a direct (histamine) and an indirect (ultrasonically ...

Safety of fluticasone propionate cream 0.05% for the treatment of severe and extensive atopic dermatitis in children as young as 3 months.

BACKGROUND Topical corticosteroids are useful for the treatment of pediatric dermatoses. However, concerns regarding possible systemic and topical toxicities have limited the use of moderate-potency corticosteroids in children. OBJECTIVE Our purpose was to characterize the safety of fluticasone propionate cream in children. METHODS Children between 3 months and 5 years 11 months (n = 32) an...

Decreased cortisol response to insulin induced hypoglycaemia in asthmatics treated with inhaled fluticasone propionate.

AIMS To assess adrenal function in asthmatic children treated with inhaled fluticasone propionate for up to 16 weeks. METHODS Children with asthma and bronchial hyperresponsiveness to inhaled methacholine were treated with inhaled fluticasone 250-750 microg/day via Volumatic spacer. The insulin tolerance test (ITT) was performed to assess adrenal function. RESULTS Eighteen asthmatic patient...

Fluticasone induces apoptosis in peripheral T-lymphocytes: a comparison between asthmatic and normal subjects.

Apoptosis is an important mechanism allowing inflammation to be limited. Glucocorticoids are the most effective anti-inflammatory agents in asthma therapy and induce cell apoptosis. Since T-lymphocytes are critically involved in airway inflammation in asthma, the effects of fluticasone propionate (FP) on apoptosis in unstimulated and in interleukin (IL)-2 stimulated peripheral blood T-lymphocyt...
