چکیده

Purpose: The clinical advantages of end-to-end (ETE) anastomosis have not been clear despite its biomechanical advantage over end-to-side (ETS) anastomosis. We compared the histomorphometric features of intimal remodeling after ETE and ETS anastomosis in a rabbit aortic bypass model. Methods: Thirty-two bypass operations, 16 with ETS and 16 with ETE anastomoses, were performed using aortic allografts of donor rabbits (15 per group) and polytetrafluoroethylene (PTFE) grafts (1 per group). To minimize bias from the immunologic response to aortic allografts or graft size, a long aortic tissue obtained from one donor was divided into 2 pieces and shared between each ETE and ETS bypass. PTFE graft bypasses, which are commonly used in clinical practice, were performed to provide comparison results for an allograft with a different compliance. Vessels were harvested at 1 day (1 per group), 5 days (1 per group), and 4 weeks (14 per group, including the PTFE bypass group) after surgery. Intimal thickening was evaluated with hematoxylin-eosin, van Gieson, immunohistochemical staining and Western blot analysis of TNF-α and proliferative cell nuclear antigen (PCNA) expression. Results: Mean intimal thickness and volume (0.721±0.047 mm, 5.734±0.387 mm vs. 0.883±0.048 mm, 9.068± 0.462 mm) and intima/media volume ratio (0.70±0.05 vs 1.08±0.06) were significantly smaller in ETE (P<0.05). Western blotting showed a marked increase in TNF-α (203.15±5.29 vs. 494.49±6.11) and PCNA concentrations (152.66±7.37 vs. 175.53±4.36) in the ETS group. Conclusion: ETE anastomosis results showed significantly decreased inflammatory reaction and volume of intimal hyperplasia, and therefore seemed to be associated with better long-term graft patency. The Effect of Estrogen on Endothelial Permeability of Aorta and the Level of Serum Nitrite Concentration in Cholesterol‑Fed Ovariectomized Rabbit

Estrogen Replacement Therapy (ERT) in postmenopausal women may decrease the risk of Coronary Artery Diseases (CAD). We hypothesized that Nitric Oxide (NO) releasing due to ERT may be the essential factor by which endothelial permeability decreases. Four groups of ovariectomized rabbits were under investigation for five weeks. Groups 1 & 4 received high cholesterol diet and other two groups (2 &...

Probucol reduces the cellularity of aortic intimal thickening at anastomotic regions adjacent to prosthetic grafts in cholesterol-fed rabbits.

Intimal hyperplasia is a persistent problem after implantation of prosthetic grafts. Although the mechanisms underlying this hyperplastic response are unknown, it has been proposed that such responses may be due to chronic vascular injury similar to that of atherogenesis. Thus, the role of oxidation was explored using the potent antioxidant drug probucol. Adult New Zealand White rabbits fed a m...

Flow waveform effects on end-to-side anastomotic flow patterns.

PURPOSE Restenosis due to distal anastomotic intimal hyperplasia, a leading cause of arterial bypass graft failure, is thought to be promoted by hemodynamic effects, specifically 'abnormal' wall shear stress patterns. The purpose of this study was to quantify the effects of flow waveform on peri-anastomotic flow and wall shear stress patterns. METHODS Blood flow and wall shear stress patterns...

Red wine inhibits monocyte chemotactic protein-1 expression and modestly reduces neointimal hyperplasia after balloon injury in cholesterol-Fed rabbits.

BACKGROUND Wine consumption decreases the risk of myocardial infarction. Intimal hyperplasia contributes to restenosis after angioplasty. Local ethanol delivery inhibits intimal hyperplasia after balloon injury in rabbit iliac and pig coronary arteries. The effects of wine consumption on intimal response and monocyte chemotactic protein-1 (MCP-1) expression were studied in cholesterol-fed rabbi...

Comparison of Two Different Arteriovenous Numerical 3D Simulation of Blood Flow

Anastomotic intimal hyperplasia caused by unphysiological hemodynamics is generally accepted as a reason for dialysis access graft occlusion. Optimizing the venous anastomosis can improve the patency rate of arteriovenous grafts. The purpose of this study was to examine, evaluate and characterize the local hemodynamics, and in particular, wall shear stresses in conventional venous endto-side an...
