Mitral Valvuloplasty for Isolated Congenital Mitral Valve Regurgitation
نویسندگان
چکیده
Mitral valve regurgitation (MR) is uncommon as an isolated lesion in congenital heart disease. In 1995, we encountered two children with congenital isolated MR complicated with heart failure, and both underwent valvuloplastic surgery. Resection and/or plication of the redundant leaflets combined with annuloplasty for the dilated annulus successfully restored the valve competency. Traditionally prosthetic valve replacements have been performed for this condition; however, as a prosthetic valve inhibits annular growth in children, a gradual progression of mitral stenosis is the main problem later in life. Valvuloplasty allows the growth of the valve apparatus, and moreover, does not require postoperatively the troublesome life-long anticoagulation therapy. Valvuloplasty in congenital MR is enabled with a precise application of reconstructive techniques, and should be the procedure of choice for pediatric patients.

SHORT TERM CLINICAL OUTCOME OF PERCUTANEOUS BALLOON MITRAL VALVULOPLASTY VERSUS SURGICAL CLOSED MITRAL COMMISSUROTOMY
BaIlon mitral valvuloplasty and closed mitral valve conunissurotomy were performed in 450 and 127 patients, respectively. Also, 254 patients with severe mitral stenosis were enrolled in a prospective randomized trial comparing the two procedures. Immediately after balloon mitral valvuloplasty, mean left atrial pressure was 11.38±3.54 mmHg, transmitral valve gradient was 1.8±2 mmHg, and mit...
متن کاملDouble-transseptal, double-balloon valvuloplasty for congenital mitral stenosis.
BACKGROUND Eight patients with severe congenital mitral stenosis underwent double transseptal, double-balloon valvuloplasty; two had isolated congenital mitral stenosis, six had additional cardiac defects, and one had previous surgical valvotomy. Ages ranged from 0.6 to 36 years (median, 9 years). METHODS AND RESULTS All procedures were tolerated well. After valvuloplasty, the left atrial a w...

Background: Percutaneous Transeptal Mitral Commisurotomy (PTMC) has been used in treating mitral stenosis with variable success, its main complication being mitral incompetence. There is a need to define the subgroup of the patients who benefit mostly from the procedure. Methods: We studied 110 patients (age 17 to 60 years mean 33.2) with mitral stenosis. PTMC was performed though femoral vein....
متن کاملFunctional classification dictates type of repair in "complex" mitral insufficiency: Application to a case of a hammock mitral valve in an adult patient.
T he hammock mitral valve is a rare congenital abnormality resulting in mitral stenosis, mitral regurgitation, or both. Although sometimes technically challenging, the repair of such lesion is possible and should be preferred to mitral valve replacement. We describe here for the first time the case of an adult patient who underwent mitral valve repair for severe mitral regurgitation related to ...
متن کامل[Mitral valve repair for mitral regurgitation].
We analyzed the results of mitral valve repair in 81 consecutive patients with severe mitral regurgitation. Of these patients, 66.6% had myxomatous degeneration, 11% ischemic disease, 8% chordal rupture, 5% congenital disease, and 3.7% endocarditis. Repair could not be achieved in five patients, and valve replacement was necessary. Six died during surgery (mortality 7%). During follow-up (mean ...
