Pharmacoinvasive strategy for ST-segment elevation myocardial infarction: wading through the treatment options.

چکیده

The contemporary management of patients with ST-segment elevation myocardial infarction (STEMI) involves a series of timely decisions, including the primary reperfusion strategy and a triage and transfer strategy for patients presenting to a facility not capable of percutaneous coronary intervention (PCI). Even for PCI-eligible patients presenting to a PCI-capable hospital, there are a myriad of choices. To start, what is the optimal thienopyridine? Which parenteral anticoagulant should be used? Is a glycoprotein IIbIIIa inhibitor needed? Then, add the questions of dosing; timing and duration of drug administration; and patient-specific factors, such as age, bleeding risk, or prior stroke. Once a culprit lesion is identified, should thrombectomy be performed? Is a drug-eluting stent (DES) or bare-metal stent best for the patient and lesion, and if a DES, which specific type? Although we have information on several of the possible combinations, we cannot extrapolate to all possible options.

Pharmacoinvasive Strategy Versus Primary Percutaneous Coronary Intervention in Patients With ST-Segment-Elevation Myocardial Infarction: A Propensity Score-Matched Analysis.

BACKGROUND The Strategic Reperfusion Early After Myocardial Infarction trial and the French Registry of Acute ST-elevation or Non-ST-elevation Myocardial Infarction 2015 suggested that pharmacoinvasive strategy compares favorably with primary percutaneous coronary intervention (PPCI). We sought to assess the clinical impact of pharmacoinvasive strategy compared with PPCI in real-world patients ...

Pharmacoinvasive therapy: the future of treatment for ST-elevation myocardial infarction.

Prompt reperfusion of ischemic myocardium is the major focus of acute treatment of patients with ST-segment elevation myocardial infarction (STEMI). Two reperfusion strategies have been developed: pharmacological and catheter based. Although these two strategies have traditionally been considered distinct and at times competing options, it is likely that care of patients with STEMI will be impr...

One-week and 6-month cardiovascular magnetic resonance outcome of the pharmacoinvasive strategy and primary angioplasty for the reperfusion of ST-segment elevation myocardial infarction.

INTRODUCTION AND OBJECTIVES Pharmacoinvasive strategy represents an attractive alternative to primary angioplasty. Using cardiovascular magnetic resonance imaging we compared the left ventricular outcome of the pharmacoinvasive strategy and primary angioplasty for the reperfusion of ST-segment elevation myocardial infarction. METHODS Cardiovascular magnetic resonance was performed 1 week and ...

STREAM and FAST-MI – Pharmacoinvasive therapy: A continued role for fibrinolysis in the primary PCI era

Data from the Strategic Reperfusion Early After Myocardial Infarction (STREAM) trial(6) and 5-year results from the French Registry of Acute ST-Elevation and Non-ST-Elevation Myocardial Infarction (FAST-MI)(7) are evaluated for further evidence on the effectiveness and safety of a pharmacoinvasive approach for patients presenting with acute ST-segment elevation myocardial infarction (STEMI).

Prehospital and interhospital delay in the treatment of patients with acute myocardial infarction with ST segment elevation and strategies to improve it from the perspective of the process owners: The importance of time

Introduction: Fibrinolytic drugs are one of the important strategies for the treatment of patients with acute myocardial infarction with ST segment elevation, especially in small centers. This study was conducted with the aim of evaluating the distance with the global standard for fibrinolytic treatment and the viewpoints of experts in this regard. Materials and Methods: This cross-sectional st...
