Trimetazidine effects on cardiac biomarkers in acute coronary syndrome

چکیده

Trimetazidine acts at the cellular level, increasing myocardial glucose oxidation and shifting substrate utilization from fatty acid to carbohydrate metabolism. In an ischemic situation, this promotes the formation of adenosine triphosphate, the main source of cardiac energy, at less oxygen cost. The benefit on myocyte necrosis can be judged by reduction in release of biomarkers such as cardiac troponin, myocardial creatine kinase, brain natriuretic peptide, and interleukin 6. These beneficial effects of trimetazidine have been recorded in patients with acute coronary syndrome, treated medically or by percutaneous intervention or coronary artery bypass graft surgery. What is most gratifying is that these effects on cellular protection are associated with clinical improvement in symptoms, left ventricular function, cardiac events, and even mortality. The importance of cardiac biomarkers in the prognosis of patients with acute coronary syndrome is clear. However, there are some controversies regarding measuring these cardiac biomarkers post angioplasty through percutaneous coronary intervention (PCI). In this study we will review the importance of increased cardiac biomarkers after elective PCI on the prognosis of patients. Also, the definit...

Cardiac enzyme in emergency medicine

Objective: Acute chest pain is an important and frequently occurring symptom in patients. Chest pain is often a sign of ischemic heart disease. Chest pain due to suspected Acute Coronary Syndrome (ACS) is responsible for a large and ijncreasing number of hospital attendances and admissions. Current practice for suspected ACS involves troponin testing 10–12 hours after symptom o...

Comparison of pregnancy-associated plasma protein-A, troponin and creatine kinase-MB levels in acute coronary syndrome

Background: Early diagnosis and proper treatment of patient with acute coronary syndrome (ACS) and ischemic heart disease are important in determining prognosis, preventing adverse effects, and may even save lives. In this study, the level of pregnancy-associated plasma protein-A (PAPP-A) in ACS patients was compared with the control group, in addition to cardiac Troponin (cTn) and creatine kin...
