CURRENT GUIDELINES: WHAT IS THE MINIMUM DURATION OF DAPT AFTER PCI? With Bare-Metal Stents

چکیده
T he need for antithrombotic therapy after percutaneous coronary intervention (PCI) has been a troublesome issue for physicians for more than 20 years. With the advent of drug-eluting stents (DES), a minimum of 12 months of dual-antiplatelet therapy (DAPT) has become the norm. However, evolving stent and drug technologies are now set to change this dogma. In this article, we focus on the perioperative management of antiplatelet therapy after PCI in patients who require noncardiac surgery. We begin by describing the current optimal management and then consider future possible solutions to this clinical dilemma.

The optimal duration of dual antiplatelet therapy after drug-eluting stent implantation: chasing a mirage.
T he optimal treatment duration of aspirin and a P2Y12 receptor antagonist after percutaneous coronary intervention (PCI) has vexed cardiologists since the introduction of drug-eluting stents (DES) approximately a decade ago. A short (2to 4-week) course of aspirin and a thienopyridine after elective implantation of a bare-metal stent became the standard of care by default from randomized trials...
متن کاملOptimal duration of dual antiplatelet therapy after percutaneous coronary intervention with drug eluting stents: meta-analysis of randomised controlled trials
Dual antiplatelet therapy (DAPT) is an essential component of treatment in patients with coronary artery disease treated with percutaneous coronary intervention (PCI). Recommendations for duration of DAPT after PCI should consider patient-specific risk, clinical presentation, stent characteristics, and procedural factors. Prolonged DAPT results in a reduction of stent thrombosis (ST) and myocar...
متن کاملOptimal Duration of Dual Antiplatelet Therapy After Implantation of Drug-Eluting Stents: Shorter or Longer?
Use of dual antiplatelet therapy (DAPT; the combination of aspirin and an inhibitor of platelet P2Y12) is the key pharmacological component in the management of acute coronary syndrome and percutaneous coronary intervention (PCI) with stent implantation, but the optimal treatment duration is still unclear. Although current guidelines recommend prescription of DAPT for at least 12 months after i...

Background and Purpose:Coronary artery Stenting is known to improve the outcome for ischemic heart disease. Many clinical studies have been performed to determine the long-term outcome of coronary Stenting and most of them were based on first-generation bare-metal Stents. The aim of this study is to evaluate long-term (24 6.4 month) results of second-generation bare metal stent. Materials an...
متن کاملCoronary stenting: A matter of revascularization
In the last few decades, the recommended treatment for coronary artery disease has been dramatically improved by percutaneous coronary intervention (PCI) and the use of balloon catheters, bare metal stents (BMSs), and drug-eluting stents (DESs). Catheter balloons were burdened by acute vessel occlusion or target-lesion re-stenosis. BMSs greatly reduced those problems holding up the vessel struc...
