Zollinger-Ellison Syndrome

چکیده
A B S T R A C T Gastric emptying and secretion, as well as intragastric volume and composition, were determined simultaneously in three patients with Zollinger-Ellison syndrome and in seven normal subjects. Gastric hypersecretion was observed in patients with ZollingerEllison syndrome and in normal subjects receiving pentagastrin. In contrast, the fraction of gastric contents emptied per minute (fractional rate of emptying) was increased in Zollinger-Ellison patients and unchanged or decreased in normal subjects receiving pentagastrin. The increased fractional rate of gastric emptying in patients with Zollinger-Ellison syndrome persisted despite abolition of gastric hypersecretion by metiamide. Thus, the increased fractional gastric emptying seen in patients with Zollinger-Ellison syndrome is not attributable to hypergastrinemia, or to gastric hypersecretion per se. Instead, it appears to be caused by an undefined nervous or humoral factor.

Zollinger-Ellison Syndrome — Presentation, Diagnosis, Therapy
Zollinger-Ellison syndrome results from a pancreatic tumor in the non-beta islet cells, which stimulates the maximal activity of the gastric acid-secreting cells, resulting in gastrointestinal ulceration of the mucosa. Multiple endocrine neoplasia type 1 (MEN 1) is a familial autosomal dominant syndrome that includes Zollinger-Ellison syndrome as part of it; however, Zollinger-Ellison syndrome ...
متن کاملGastric acid secretion and diagnosis of Zollinger-Ellison syndrome.
One hundred and fifty patients suspected of having Zollinger-Ellison syndrome were investigated. High plasma gastrin levels were found in 23 and in 16 a diagnosis of Zollinger-Ellison syndrome was confirmed. Measurement of the basal and maximal acid outputs showed that these simpler investigations have considerable value as screening tests for the selection of those patients in whom gastrin stu...
متن کاملIs total gastrectomy still a viable option in the management of patients with the Zollinger-Ellison syndrome?
The Zollinger-Ellison syndrome, which is due to the secretion of gastrin from non-beta islet cell tumors of the pancreas, results in gastric hypersecretion and severe peptic ulcer disease. In the 1970s H2 blockers became the mainstay of therapy. More recent evidence indicates that early surgical intervention aimed at tumor extirpation results in improved outcome. The purpose of this paper is to...
متن کاملCimetidine in the management of the Zollinger-Ellison syndrome.
The first case of the Zollinger-Ellison syndrome treated from the outset continuously for more than a year with the histamine H2-receptor antagonist cimetidine is reported. Symptomatic response has been dramatic and sustained, marked endoscopic improvement has been observed with healing of oesophageal, gastric and stomal ulcers.
