Iodide mumps: acute sialadenitis after contrast administration for angioplasty.

چکیده
A63-year-old black woman with hypertension, diabetes mellitus, and end-stage renal disease who was on dialysis underwent percutaneous intervention of a right coronary artery. A total of 100 cc of Optiray (74% Ioversol, a nonionic low-osmolar contrast agent with 350 mg/mL of organically bound iodine) was used during the procedure. A few hours after the procedure, the patient noticed swelling and pain in the upper neck region. She recalled a history of mumps in childhood and was on aspirin, ticlopidine, losartan, and thyroxin. Physical examination revealed an afebrile patient with a regular heart rate of 75 bpm and a blood pressure of 109/ 50 mm Hg. Bilateral, diffuse submandibular gland enlargement and mild parotid gland enlargement were noted (Figure 1). Oral mucosa was dry without erythema, abscess, or ulcers. Contrastinduced acute sialadenitis (iodide mumps) was suspected on the basis of clinical presentation. Treatment with analgesics and dialysis led to complete resolution of submandibular gland swelling (Figure 2). She has had no recurrence of sialadenitis and has remained asymptomatic since discharge.

Iodide mumps is an uncommon condition induced by iodide-containing contrast. We present the first reported case of iodide mumps in mainland China, which occurred after carotid artery intervention. The patient, a 65-year-old Chinese male, had a history of dizziness, hypertension, diabetes, and right arm weakness. He had no history of allergies and had never previously received iodide-containing c...

To the Editor: We present 2 cases of bilateral swelling of the parotid glands appearing after application of contrast media. This unusual reaction is also known as Biodide mumps.[ As there is no premedication that can prevent its appearance, it is advisable in these cases to use imaging systems without iodide-based contrast media. The first case was a 70-year-old man with significant bilateral,...

Relation: Iodide mumps is an uncommon condition, induced by iodide-containing contrast, and is characterized by a rapid, painless enlargement of the bilateral or unilateral salivary gland. At present, the pathogenesis of iodide mumps is not yet clear. It may be related to an idiosyncratic reaction, a toxic accumulation of iodine in the gland duct, or renal function damage leading to an iodine e...
متن کاملA Complete Resolution of Sialadenitis Induced by Iodine Containing Contrast with Intravenous Dexamethasone Infusion
Salivary gland enlargement following the administration of iodine is an extremely rare event, and the pathophysiology of iodine-induced sialadenitis is not yet fully known. The onset of symptoms can start within a few minutes to five days after contrast administration. The course of iodine-induced sialadenitis is extremely benign, and rapid resolution of symptoms is expected without treatment. ...
