Pressure Tracings in Hypertrophic Cardiomyopathy With Dynamic Midventricular Obstruction

چکیده
A 50-year-old man was admitted for evaluation of syncope. Clinically, he had an irregular pulse and an ejection systolic murmur loudest at the left sternal border. Transthoracic echocardiography revealed hypertrophic cardiomyopathy (HCM) with midventricular obstruction and apical aneurysm with maximum pressure gradient greater than 74 mm Hg. Coronary angiogram showed normal coronary arteries. Left ventriculogram using 4 Fr pigtail catheter via right radial approach showed nearly total obliteration of left ventricular (LV) cavity at the midventricular level (arrows) with apical aneurysm (Fig. 1A and B). Pressure tracing from LV apex to the aorta showed a significant systolic pressure gradient within the left ventricular cavity (Fig. 1C). The day after cardiac catheterization, he complained of palpitation and mild dizziness and an electrocardiogram showed sustained monomorphic ventricular tachycardia (VT) of LV apical origin. The final diagnosis was HCM with midventricular obstruction complicated by VT. HCM is an extremely heterogeneous disease. In a patient with Images in Cardiovascular Medicine

Midventricular Obstructive Hypertrophic Cardiomyopathy during Pregnancy Complicated by Pulmonary Embolism: A Case Report
Hypertrophic cardiomyopathy (HCM) with midventricular obstruction (MVO) is a rare condition occurring in 1% of HCM patients. It is characterized by asymmetric left ventricular hypertrophy with MVO and elevated intraventricular pressure gradients. Pulmonary embolism has been associated with mid-ventricular obstructive HCM. Briefly, this case presents an unusual clinical scenario where a young pr...
متن کاملHemodynamic Consequences of Hypertrophic Cardiomyopathy with Midventricular Obstruction: Apical Aneurysm and Thrombus Formation
BACKGROUND Hypertrophic cardiomyopathy (HCM) with midventricular hypertrophy is an uncommon phenotypic variant of the disease. Midventricular hypertrophy predisposes to intracavitary obstruction and downstream hemodynamic sequelae. CASE REPORT We present a case of HCM with midventricular hypertrophy and obstruction diagnosed after a CT scan of the abdomen incidentally revealed a filling defec...

Obstructive Hypertrophic Cardiomyopathy: Pathogenesis of the Obstruction In most patients, the hypertrophy develops initially in the septum and extends to the free walls, often giving a picture of concentric hypertrophy. In '25% of patients with familial hypertrophic cardiomyopathy (FHCM), asymmetrical septal hypertrophy leads to a highly variable pressure gradient between the apical left ventr...

BACKGROUND Subaortic and midventricular hypertrophic cardiomyopathy in a patient with extreme segmental hypertrophy exceeding the usual maximum wall thickness reported in the literature is a rare phenomenon. CASE PRESENTATION A 19-year-old man with recently diagnosed hypertrophic cardiomyopathy (HCM) was referred for sudden death risk assessment. The patient had mild exertional dyspnea (New Y...
متن کاملSubaortic and mid-ventricular obstructive hypertrophic cardiomyopathy with an apical Aneurysm: a case report
BACKGROUND Most patients with hypertrophic cardiomyopathy (HCM) have asymmetric septal hypertrophy and among them, 25% present dynamic subaortic obstruction. Apical HCM is unusual and mid-ventricular HCM is the most infrequent presentation, but both variants may be associated to an apical aneurysm. An even more rare presentation is the coexistence mid-ventricular and apical HCM. This case is a ...
