Urinary Dickkopf-3 and Contrast-Associated Kidney Damage

چکیده
Administration of iodinated contrast medium (CM) during invasive cardiovascular procedures may be associated with impairment kidney function. Urinary dickkopf-3 (DKK3), a stress-induced renal tubular epithelium–derived glycoprotein, has been identified as biomarker predicting both acute injury (AKI) and persistent dysfunction. DKK3/creatinine ratio (uDKK3/uCr), urine serum neutrophil gelatinase-associated lipocalin (uNGAL, sNGAL) cystatin C (sCyC) were assessed in 458 patients chronic disease scheduled for requiring CM administration universal adoption nephroprotective interventions. Contrast-associated AKI (CA-AKI) was defined creatinine increase ?0.3 mg/dl at 48 h after administration. Persistent dysfunction estimated glomerular filtration rate reduction ?25% 1 month compared baseline. CA-AKI occurred 64 or the (14%), baseline uDKK3/uCr ?491 pg/mg best threshold its prediction. Net reclassification improvement (NRI) significantly increased by adding to Mehran, Gurm, National Cardiovascular Data Registry (NCDR) scores (all p < 0.05), same applied integrated discrimination (IDI) when Gurm NCDR (p 0.001). 57 (12%) ?322 appeared Adding IDI NRI Baseline seems reliable marker improving identification undergoing coronary peripheral risk

متن کاملPredictive Values of Urinary Interleukin 18 and Neutrophil Gelatinase-Associated Lipocalin for Delayed Graft Function Diagnosis in Kidney Transplantation
Background: Delayed graft function is a main complication after deceased donor kidney transplantation that adversely affects graft outcome. Difficulties in prediction and early detection of delayed graft function have hindered the ability to perform proper therapeutic interventions. We investigated whether measuring urinary interleukin 18 and neutrophi...
متن کاملContrast-Induced Nephropathy Following CT Scan and Its Associated Factors
Background and purpose: Contrast-induced nephropathy (CIN) as a common complication of administration of contrast media (CM) is a major source of hospital morbidity and mortality. The aim of this study was to determine the incidence of CIN after contrast-enhanced CT scan and identifying the related risk factors. Materials and methods: A cross-sectional study was carried out in 100 patients att...
متن کاملAssessing the Prediction Power of Plasma Neutrophil Gelatinase-Associated Lipocalin and Serum Cystatin C for Diagnosis Kidney Damage
Background & Objective: Chronic Kidney Disease (CKD) has been recognized as a serious public health threat. The early detection of kidney damage in CKD is a useful way to reduce the disease burden. This study aimed to determine the power of Neutrophil gelatinase-associated lipocalin (NGAL) and cystatin C (Cys-C) to predict the kidney damage in Iranian patients. Materials & Methods: This st...
متن کاملNeutrophil Gelatinase-Associated Lipocalin and Contrast-Induced Acute Kidney Injury.
BACKGROUND Neutrophil gelatinase-associated lipocalin (NGAL) is an early marker of acute kidney injury (AKI). METHODS AND RESULTS Urine NGAL and serum NGAL (sNGAL) were assessed at 2, 6, 24, and 48 hours after contrast media (CM) exposure in 458 high-risk patients (development set). Optimal thresholds in predicting contrast-induced AKI (serum creatinine [sCr] increase ≥0.3 mg/dL at 48 hours a...
