TIPS-10 FEASIBILITY OF A MULTI-MODALITY CENTRAL NERVOUS SYSTEM SCREENING EVALUATION IN HER2+ BREASTCANCER PATIENTS

Abstract BACKGROUND Breast cancer is the most common in world and second to metastasize central nervous system (CNS). CNS-metastases are type of brain tumor have a rising incidence. They associated with neurologic morbidity mortality remain major clinical challenge. Survival withCNS-metastases short median survival order months. The incidence 5.1% breast however true prevalence not known; at autopsy this ranges between 20-40%. One contributing factor towards development HER2-positivity. Between 25%-48% patients HER2-positive metastatic develop CNS-metastases. Currently, routine CNS surveillance imaging recommended, guidelines suggest having low threshold for evaluation and/or cerebrospinal fluid (CSF) analysis because high rate relevance occult CNS-metastases, after identification importance earlier initiation therapy unknown. Existing treatment strategies rely on combinatorial approach chemotherapy, radiotherapy surgery which typically affords limited disease control. This trial seeks study if CSF feasible as early detection mechanisms. METHODS two-cohort establish feasibility multi-modality surveillance. Patients HER2+ (with no known CNS-metastases) who either Stage IV (Cohort A) or II/III eligible B). On study, will undergo MRI Brain 6 monthly intervals. includes cytology, circulating cells cell-free DNA analysis. We plan enroll 20 (10 onto each cohort). 1 enrolled. first screening cancer.
