: Air plombage is a surgical procedure that reduces the pleural space. In 1953, Dr. Chamberlain designed original “extraperiosteal plombage” to prevent complications after pulmonary segmentectomy for tuberculosis. This technique was developed formation of an intrapleural pocket dead space, and allow collapse cavity obliteration extrapleural Thus, this has been called as “(extraperiosteal) air plombage.” Japan, modified “Kinchu method” performed. The indication follows: (I) remaining parenchyma resection does not expand fill residual (II) large raw surfaces remain in plane, which cannot easily contact chest wall, (III) there excessive leak from parenchyma. Surgical portion ribs above space are stripped subperiosteally, leaving attached periosteum on their outer surfaces. size, shape, location “resultant pocket” made defect left by lung resection. pleura, periosteum, intercostal muscles facia drop into resected surface However, care should be taken with regards pre-, intra-, post- operation. Particularly, minimize atrophic deformative changes late phase plombage, it important avoid dissecting costal lower (the 8th–12th) ribs.
