Primary aortoenteric fistula (PAEF) is spontaneous development of communication between the native aorta and anywhere within gastrointestinal tract. It extremely rare fatal condition which usually presents as a painless upper bleed. This often overlooked because it's rarity low index suspicion by physicians despite availability wide range diagnostic tools. Computed tomography angiography (CTA) most common investigation done to diagnosis PAEF. paper reports case 49 years old female with massive (GI) bleeding. A PAEF was diagnosed CTA called for an emergency laparotomy surgical repair synthetic vascular bypass graft. The patient recovered well.
