چکیده

Catheter ablation within the right atrium can be limited by proximity to phrenic nerve and potential for hemi diaphragmatic paralysis. High output pacing is frequently employed map position relative endocardium, particularly in relation superior vena cava pulmonary vein. The use of intracardiac echocardiography (ICE) correlation with lower rigorously course along has not been previously described. We describe a case incessant atrial tachycardia successfully mapped ablated close using combination ICE imaging medium-output nerve. n/a An 18-year-old man was referred repeat following unsuccessful attempt at an outside institution. Prior transient palsy cryoablation mid crista terminalis. Epicardial access deflect anticipated. Atrial endocardially prior obtaining epicardial access. catheter advanced SVC used identify integrated into CARTO navigational system (Biosense Webster). Pacing performed 10 mA further refine atrium. Regions anatomic loss pace-capture correlated sites physiologic effusion, where pericardial fluid concentrated separation from visceral epicardium noted. After mapping nerve, endocardial absence injury without need Phrenic may prevented rigorous ultrasound pacing.

Left atrial thrombus associated with ablation for atrial fibrillation: identification with intracardiac echocardiography.

OBJECTIVES This study reports the incidence of, risk factors for, and management of left atrial (LA) thrombus documented by intracardiac echocardiography (ICE) during LA ablation for atrial fibrillation (AF). BACKGROUND Thrombus formation is a risk associated with LA ablation procedures. METHODS Intracardiac echocardiography imaging was performed in 232 patients (184 men, average age 55 +/-...

Phrenic Nerve Injury After Catheter Ablation of Atrial Fibrillation

UNLABELLED Phrenic Nerve Injury (PNI) has been well studied by cardiac surgeons. More recently it has been recognized as a potential complication of catheter ablation with a prevalence of 0.11 to 0.48 % after atrial fibrillation (AF) ablation. This review will focus on PNI after AF ablation. Anatomical studies have shown a close relationship between the right phrenic nerve and it's proximity to...

Left ventricular pacing with long pulse duration can avoid phrenic nerve stimulation.

BACKGROUND One of the major limitations of left ventricular pacing is phrenic nerve stimulation. OBJECTIVE We evaluated the characteristics of the strength-duration curves of the left phrenic nerve (PN) and epicardium of the left ventricle (LV). METHODS A total of 12 mongrel dogs were studied. Capture threshold was assessed during pacing with 180 beats/min and various pulse durations (0.25 ...

Right atrial thrombus aspiration guided by intracardiac echocardiography during catheter ablation for atrial fibrillation.

A 56-year-old man with psoriasis and a 2-year history of paroxysmal atrial fibrillation and flutter was admitted for catheter ablation. The procedure was performed under general anesthesia. The patient arrived to the electrophysiology laboratory in atrial flutter. A preprocedural transesophageal echocardiogram confirmed the absence of intraatrial thrombus. The patient was chronically anticoagul...

Use of intracardiac echocardiography to guide ablation of atrial fibrillation.

Intracardiac Echocardiography Ultrasound technology has progressed rapidly over the past several years, leading to significant miniaturization of transducer design. The original catheter-based transducers, known as intravascular ultrasound (IVUS) catheters, are high frequency (20 to 40 MHz) and are used to image the walls of coronary arteries and other small vessels. They contain a single ultra...
