Impact of Coronary Artery Disease and Percutaneous Coronary Intervention on Transcatheter Aortic Valve Implantation

چکیده
The prevalence of coronary artery disease (CAD) detected in preoperative work-up for transcatheter aortic valve implantation (TAVI) is high. Instead, the management a concomitant CAD and its effect on procedural outcomes remain unclear. We evaluate impact revealed during workup percutaneous intervention (PCI) TAVI procedures. A retrospective study was conducted 1336 consecutive patients who underwent Toulouse University Hospital, Rangueil, France. studied population divided into 2 groups: CAD-TAVI group No group. Then, segregated subgroups: PCI-TAVI In-hospital adverse clinical were assessed each Pre-TAVI significant 36% 1030 eligible inclusion study. overall in-hospital death, stroke, major or life threatening bleeding, minor vascular complications, pacemaker implantation, acute kidney injury 2.7%, 2.4%, 2.8%, 3.6%, 3.9%, 7.5%, 12.5%, respectively. Among population, 55% admitted to cardiac care unit. statistical difference observed between groups. Patients with diagnosed pre-TAVI not more likely develop post-TAVI procedure. Also, no mortality observed. In parallel, performing PCI did increase risk death complications. terms distribution antithrombotic regimen may explain higher non bleeding events

Coronary Artery Disease and Symptomatic Severe Aortic Valve Stenosis: Clinical Outcomes after Transcatheter Aortic Valve Implantation
INTRODUCTION The impact of coronary artery disease (CAD) on outcomes after transcatheter aortic valve implantation (TAVI) has not been clarified. Furthermore, less is known about the indication and strategy of revascularization in these high risk patients. AIMS This study sought to determine the prevalence and prognostic impact of CAD in patients undergoing TAVI, and to assess the safety and ...
متن کاملCoronary Obstruction Following Transcatheter Aortic Valve Implantation
BACKGROUND Transcatheter aortic valve implantation (TAVI) was established as an important alternative for high-risk patients with severe aortic stenosis. However, there are few data in the literature regarding coronary obstruction, that although rare, is a potentially fatal complication. OBJECTIVE Evaluate this complication in Brazil. METHODS We evaluated all patients presenting coronary ob...
متن کاملSuccessful percutaneous coronary intervention after transcatheter aortic valve implantation with CoreValve bioprosthesis
We present a case of an 84-year-old male patient who was admitted to our department due to recurrent angina 17 months after transcatheter aortic valve implantation (TAVI) with a 29 mm CoreValve bioprosthesis (Figure 1). Symptoms were increasing in the past several weeks, and on admission the patient was in Canadian Cardiovascular Society class 3. His past medical history included hypertension a...
متن کاملCombining Transcatheter Aortic Valve Replacement and Coronary Angiography/Percutaneous Coronary Intervention Procedures: Ready for Prime Time?
About half of elderly patients with severe aortic stenosis exhibit some degree of obstructive coronary artery disease that is routinely addressed at the time of surgical aortic valve replacement (SAVR). Transcatheter aortic valve replacement (TAVR) has emerged as a valid alternative to SAVR for the treatment of intermediate to high surgical risk patients. More than half of TAVR candidates have ...
متن کاملAcute myocardial infarction shortly after valve-in-valve transcatheter aortic valve implantation successfully managedwith challenging percutaneous coronary intervention.
In recent years, transcatheter aortic valve implantation (TAVI) has been considered a novel option for the management of surgically high-risk patients requiring aortic valve replacement. Presently described is a case of acute coronary syndrome (ACS) managed with a challenging primary percutaneous coronary intervention (PCI) shortly after a valve-in-valve TAVI intervention. This case highlights ...
