Experimental long-term sub-normothermic machine perfusion for non-allocable human liver grafts: first data towards feasibility

Summary Background Patients with end-stage liver disease can only be cured by transplantation. Due to the gap between demand and supply, surgeons are forced use expanded criteria donor (ECD) organs, which more susceptible ischemia–reperfusion injury (IRI). Therefore, enhanced storing techniques required. Machine perfusion (MP) has moved into spotlight of research because its feasibility for investigating function prior implantation. However, as perfect MP protocol not yet been found, we aimed investigate potential sub-normothermic (SN)MP in this field. Methods Non-allocable human livers were subjected 24 h SNMP at 21 °C after delivery study team. Perfusion was performed Custodiol® (Dr. Franz Köhler Chemie, Bensheim, Germany) or Belzer MPS® (Bridge Life Europe, London, UK) perfusate parameters determined. For determination biliary conditions, pH, glucose, HCO 3- levels measured. Results Liver slightly increased irrespective reason rejection during perfusion. Six failed produce bile completely, whereas remaining 10 produced 2.4 ml 179 bile. Biliary carbonate all but one liver. The bile-glucose-to-perfusate-glucose ratio near 1 most organs pH above 7 case. Conclusion This provides promising data on long-term a tool gain time optimize ECD decrease organ supply. Long-term (24 h) machine seems possible, although some adjustments might necessary improve general outcome. so far shown normothermic perfusion, bearing drawbacks compared variant.
