Cisplatin-induced Nephrotoxicty and its Predisposing Factors for Unrecovered Kidney Injury: A Report from Indonesia National Cancer Center

چکیده
BACKGROUND: Cisplatin-induced nephrotoxicity is the most common and devastating side effects which affects long-term outcome. It may be reversible in some patients but also cause permanent kidney injury. AIM: This research aims to evaluate risk factors of cisplatin unrecovered function at national cancer hospital Indonesia. METHODS: a retrospective cohort study conducted Dharmais National Cancer Hospital, Jakarta, All aged 18 years old above, who received cisplatin-based regimen as first-line chemotherapy completing all cycles with least four cycles, were included study. Demographic data collected was evaluated using estimated glomerular filtration rate (eGFR) before, 3 times during chemotherapy, 1 months after last dose. RESULTS: A total 177 analysis, mean age 45.80 ± 11.75 old, majority diagnosed nasopharynx (58.8%), have baseline eGFR 102.76 20.68 mL/min 1.73m2. Nephrotoxicity occurred 80 (45.2%) 27 (15.3%) toxicity first cycle. Age above 50, hypertension non-steroidal anti-inflammatory drugs (NSAID) use associated increased cisplatin-induced nephrotoxicity. include 50 (OR 4.18, 95% CI 2.11–8.28; p < 0.0001), 2.03, 1.03–4.01; = 0.040), NSAID 2.34, 1.22–4.93; 0.025). Risk 75% 0.47, 0.17–0.56; 0.001) gender 0.018, 0.03–0.95; 0.043). CONCLUSION: occurs 45.2% throughout cycles. includes hypertension, while female are factor for not recover

Hyperhomocysteinemia Exacerbates Cisplatin-induced Acute Kidney Injury
Hyperhomocysteinemia (HHcy) has been linked to several clinical manifestations including chronic kidney disease. However, it is not known whether HHcy has a role in the development of acute kidney injury (AKI). In the present study, we reported that HHcy mice developed more severe renal injury after cisplatin injection and ischemia-reperfusion injury shown as more severe renal tubular damage an...
متن کاملPathophysiology of Cisplatin-Induced Acute Kidney Injury
Cisplatin and other platinum derivatives are the most widely used chemotherapeutic agents to treat solid tumors including ovarian, head and neck, and testicular germ cell tumors. A known complication of cisplatin administration is acute kidney injury (AKI). The nephrotoxic effect of cisplatin is cumulative and dose-dependent and often necessitates dose reduction or withdrawal. Recurrent episode...
متن کاملCXCL16 regulates cisplatin-induced acute kidney injury
The pathogenesis of cisplatin-induced acute kidney injury (AKI) is characterized by tubular cell apoptosis and inflammation. However, the molecular mechanisms are not fully understood. We found that CXCL16 was induced in renal tubular epithelial cells in response to cisplatin-induced AKI. Therefore, we investigated whether CXCL16 played a role in cisplatin-induced tubular cell apoptosis and inf...
متن کاملSuramin protects from cisplatin-induced acute kidney injury.
Cisplatin, a commonly used cancer chemotherapeutic, has a dose-limiting side effect of nephrotoxicity. Approximately 30% of patients administered cisplatin suffer from kidney injury, and there are limited treatment options for the treatment of cisplatin-induced kidney injury. Suramin, which is Federal Drug Administration-approved for the treatment of trypanosomiasis, improves kidney function af...
متن کاملIncidence of cisplatin-induced nephrotoxicity and associated factors among cancer patients in Indonesia.
BACKGROUND Cisplatin is still used as a first-line medication for solid tumors. Nephrotoxicity is a serious side effect that can decrease renal function and restrict applicable doses. This research aimed to obtain the profile of cisplatin-induced nephrotoxicity and its associated factors in adult cancer patients at Dharmais National Cancer Hospital (DNCH). MATERIALS AND METHODS The design was...
